Visit details

Primary Care does not have to begin and end inside a medical office. You can build an ongoing relationship with a board-certified Family Nurse Practitioner who manages new illnesses, routine visits, screenings, chronic conditions, testing and coordination with your other clinicians.

Over time that relationship pays off. Your provider learns your medical history, current medications, past lab results, how you responded to treatment and what you want from your health. Changes are easier to spot, and you stop starting over with a new provider every time something comes up.

Primary Care

Conditions we commonly evaluate and treat

Examples, not a complete list. If you do not see your concern, ask us.

Four kinds of care, one provider

1

Acute Care

New illnesses, symptoms and short-term concerns that can be handled safely by video.

2

Routine Care

Regular visits, medication reviews, follow-up testing and screening coordination.

3

Preventive Care

Spotting risks early, recommending the right screenings and watching risk factors over time.

4

Chronic Care

Hypertension, diabetes, cholesterol, thyroid disorders, asthma, weight, mental health and sleep, managed over months and years.

What a primary care visit can include

  • Medication management
  • Preventive screening recommendations
  • Routine and diagnostic lab work
  • Hormone testing when clinically appropriate
  • PSA testing when appropriate
  • Imaging orders
  • Sleep-study coordination
  • Specialist consultations and referrals
  • Follow-up after urgent care, the ER or a hospital stay
  • Coordination with your other clinicians
More Than a Video Visit

Your care doesn’t end when the video visit ends.

Wade’s Care First is a full medical practice delivered through telehealth. When it is medically appropriate, we order, prescribe, refer and coordinate the services you would expect from a traditional outpatient practice.

Patient Questions

Questions about primary care.

All Patient Questions
How can home blood pressure readings be used during a virtual visit?

Bring several readings with dates and times if available. We consider the trend, symptoms and your medications before deciding whether monitoring, testing or a treatment change is appropriate.

Can primary care help when I have several symptoms that may be connected?

Yes. We review symptoms together rather than treating each one in isolation, and recommend laboratory testing, examinations or specialist input when needed.

How do you follow diabetes between appointments?

Follow-up may include home glucose readings, A1c and kidney tests, medications, nutrition and symptoms. The testing schedule is individualized.

Can you reassess my thyroid medication if I am still tired?

Yes. We can review your symptoms and medication use and order thyroid or other tests when indicated. Fatigue has many possible causes, so a dose change is not always the answer.

Can you evaluate recurring acid reflux or heartburn?

Yes. We can discuss triggers, medicines, diet and warning symptoms. Trouble swallowing, bleeding or certain persistent symptoms may require in-person evaluation.

What if I have frequent headaches or migraines?

We can evaluate recurring headache patterns, possible triggers and treatment response. A sudden, unusually severe headache or new weakness, trouble speaking, vision changes or other neurological symptoms needs emergency care. Do not wait for a routine video appointment.

Can you help manage mild asthma between flare-ups?

Yes. We can review symptom frequency, inhaler technique, triggers and an appropriate action plan. Significant breathing difficulty requires urgent in-person or emergency care.

How do you approach persistent fatigue when routine tests are normal?

We consider sleep, mood, medications, nutrition and other medical factors, then decide whether further evaluation or referral is warranted.

Can we address arthritis symptoms during a primary care visit?

Yes. We can review symptoms and function, discuss treatment and activity strategies, and arrange imaging or in-person assessment if needed.

Will my long-term treatment plan change as my health changes?

Yes. Ongoing primary care involves reassessing goals, diagnoses, medications and test results over time, rather than using a fixed plan indefinitely.

Can Wade’s Care First be my primary care provider?

Yes. For many patients, Wade’s Care First serves as an ongoing Primary Care Provider through telehealth for Acute Care, Routine Care, Preventive Care, and Chronic Care. That includes labs, imaging orders, screenings, medication management and specialist referrals.

Can a Family Nurse Practitioner provide primary care?

Yes. Family Nurse Practitioners are trained and board certified to provide primary care for patients across the lifespan, including diagnosing and treating illness, ordering tests, prescribing medication and managing chronic conditions within state law.

Can I receive ongoing primary care through telehealth?

Yes. You can see the same provider over time for routine visits, chronic disease management, screenings and follow-up. That continuity is what lets your provider notice changes and keep your care connected.

Can I switch my primary care to Wade’s Care First?

Yes. Book a primary care visit to establish care and, if you can, share records from your previous provider through the patient portal. We review your history, medications and screenings and build your plan from there.

Can I use Wade’s Care First if I already have another primary care provider?

Yes. Many patients use us for faster sick visits, mental health care or specific services while keeping another primary care provider. With your permission, we can share notes so everyone stays informed.

Good to know

Services, screening recommendations, testing, treatment options, referrals and prescribing may vary with your age, medical history, symptoms, where you are physically located at the time of the visit, state and federal requirements, insurance requirements, and whether your condition can be safely managed by telehealth. Conditions and services listed here are examples. An appointment does not guarantee a particular diagnosis, a prescription, a controlled medication, hormone therapy, a lab order, an imaging study, a sleep study, a specialist referral, an accommodation letter or a particular treatment. Treatment decisions follow a clinical evaluation and the provider’s judgment.
