1Individualized Care
Care based on your health needs, medical history, lifestyle, goals and preferences, not a single symptom. The plan is built for you, and it changes as you do.
From his first days as a CNA to founding Wade’s Care First, Charles has built his career on one belief: the patient stays at the center of their care.

Charles grew up in Gary, Indiana, and began his healthcare career as a Certified Nursing Assistant. Working at the bedside taught him a lesson that has followed him ever since. Sometimes the most important thing a healthcare professional can do is listen.
He went on to become a Registered Nurse in neuro-trauma and later the float pool. Moving between units, conditions and levels of acuity gave him a wide view of healthcare and a deeper respect for treating each patient as an individual.
As a Family Nurse Practitioner, he widened that experience further through hospitalist medicine, service as a Medical Officer of the Day, and primary care for veterans.
The roles changed. The principle stayed the same.

A telehealth practice built to make care easier to reach while keeping the relationship between patient and provider personal.
Working across bedside nursing, hospital medicine and primary care showed Charles what healthcare does well and where patients can be left behind. It also shaped the provider he wanted to be.
He created Wade’s Care First because he believed care could be more accessible, more personal and more focused on the person receiving it. Seeing healthcare as a CNA, an RN and a Nurse Practitioner reinforced a simple idea: treating a patient means looking past a single diagnosis or complaint.
Here, patient-centered care starts with listening. It means understanding your concerns and goals, considering the whole person, explaining options clearly, answering questions and involving you in decisions about your health. Telehealth extends that mission by bringing quality care closer while keeping the relationship personal.
For Charles, good care is measured by one thing. Does the patient leave feeling heard, respected, informed and confident about what comes next?
“My goal is simple: to treat every patient the way I would want my own mother treated when she visits a healthcare provider.”
That means taking concerns seriously and listening before making assumptions. It means explaining care clearly so nobody leaves confused. And it means remembering that behind every appointment is a person placing their trust in a healthcare professional.
Three commitments from the first version of Wade’s Care First, carried into everything we do today.
1Care based on your health needs, medical history, lifestyle, goals and preferences, not a single symptom. The plan is built for you, and it changes as you do.
2Your background, values, beliefs and preferences are respected and considered as part of your care. You should feel understood, not processed.
3Physical health, mental health, prevention, chronic conditions, lifestyle and long-term wellness, looked at together rather than one symptom in isolation.
What is being recommended and why, in plain language.
Ask anything. There is time for it in the visit.
What worries you shapes the visit.
Whenever appropriate, choices about your health are made together with you.
This is the standard for Wade’s Care First: Listen to the patient. Understand the whole person. Keep the patient at the center of care.
