Work and school documentation

Medical accommodation and documentation visits

If a medical condition affects your work or schooling, a documentation visit evaluates whether an accommodation letter is supported by your history and clinical findings.

Documentation visits are for established Wade’s Care First patients. New patients need the appropriate initial clinical evaluation first.

Visit details

Documentation is provided only when the clinical evaluation, available medical information and the provider’s professional judgment support it. Bring any records that describe your condition and how it affects your day.

Booking this visit does not guarantee that a requested letter, restriction, certification or accommodation will be approved.

Patient Questions

Questions about accommodation letters.

All Patient Questions
What kinds of functional limitations can an accommodation evaluation discuss?

We can review clinically supported limitations affecting work or school activities. Documentation, if issued, must reflect the findings of an appropriate medical evaluation.

Does a diagnosis automatically qualify me for a particular accommodation?

No. Requests depend on the functional impact of a condition and the relevant workplace or school process. A diagnosis alone does not determine an outcome.

Can you guarantee an accommodation letter after one visit?

No. Whether documentation can be provided depends on clinical findings and sufficient supporting information.

Should I upload a school or employer form before my appointment?

If available, bring the form, deadline and requested information so we can determine whether an evaluation and documentation are appropriate.

Can a letter explain how symptoms affect daily tasks?

When medically supported, documentation may describe relevant functional limitations rather than unnecessary private medical details.

Can I request a letter based only on a self-reported diagnosis?

A clinical assessment and, when needed, supporting records are required. We cannot certify findings that have not been adequately evaluated.

Will my employer or school receive my full medical record?

Not automatically. Any disclosure follows applicable privacy and consent requirements; documentation should generally be limited to what is appropriate for the purpose.

Can you complete disability-benefit or legal forms through this visit?

Not necessarily. Some forms require a specialized examination, established treatment history or other records; we assess whether the request falls within the service's scope.

Can accommodation documentation be backdated?

We cannot promise retroactive or unsupported statements. Dates and descriptions must accurately reflect available clinical information.

Who decides whether my accommodation is approved?

The employer, school or other reviewing organization makes that determination. Wade's Care First provides clinical documentation only when appropriate.

Good to know

Services, screening recommendations, testing, treatment options, referrals and prescribing may vary with your age, medical history, symptoms, where you are physically located at the time of the visit, state and federal requirements, insurance requirements, and whether your condition can be safely managed by telehealth. Conditions and services listed here are examples. An appointment does not guarantee a particular diagnosis, a prescription, a controlled medication, hormone therapy, a lab order, an imaging study, a sleep study, a specialist referral, an accommodation letter or a particular treatment. Treatment decisions follow a clinical evaluation and the provider’s judgment.
