Elko County is a federally designated mental health HPSA (all rural and frontier Nevada residents live in one) and is among the rural counties designated as single-county primary care HPSAs per UNR Med’s 2025 summary of HRSA data. Telehealth can complement local clinicians by offering another way to access primary care, mental health and medication follow-up while you keep your local relationships.

Services, screening recommendations, testing, treatment options, referrals and prescribing may vary with your age, medical history, symptoms, where you are physically located at the time of the visit, state and federal requirements, insurance requirements, and whether your condition can be safely managed by telehealth. Conditions and services listed here are examples. An appointment does not guarantee a particular diagnosis, a prescription, a controlled medication, hormone therapy, a lab order, an imaging study, a sleep study, a specialist referral, an accommodation letter or a particular treatment. Treatment decisions follow a clinical evaluation and the provider’s judgment.

The largest town on Interstate 80 between Salt Lake City and Reno, at about 5,100 feet, with an economy built on gold mining, ranching and highway travel.
Some specialized services require travel beyond Elko, and the cited rural Nevada workforce report lists three psychiatrists and four dermatologists across the rural region. If you work a mine rotation or ranch schedule, taking a day off for a routine appointment is a real cost. A video visit with Wade’s Care First brings primary care, mental health and ADHD treatment, skin evaluations, weight management and sleep care to your phone or laptop at home, in town or at the end of a shift.
The practice is telehealth only, with no Elko office. When you need blood work, an X-ray or a prescription, the order goes to Northeastern Nevada Regional Hospital, the Quest or Labcorp site in town, or your local pharmacy.
Nevada’s 14 rural and frontier counties, with about 302,794 residents, have only 244 active physicians (3.3 percent of the state total), including 3 psychiatrists, 2 child and adolescent psychiatrists, 4 dermatologists and 11 general pediatricians across the entire rural region. Twelve of those 14 counties are single-county primary care HPSAs and 100 percent of rural and frontier residents live in a mental health HPSA. The state Primary Care Office notes travel time to a provider in rural and frontier areas can be several hours, and the state’s broadband office says gaps remain concentrated in rural and remote areas.
Nevada’s 2025 UNR Med physician workforce report finds the state would need 2,303 more physicians to reach the national per-capita average, including 251 family medicine doctors, 411 pediatricians and 105 psychiatrists; in 35 of 37 AMA specialty areas Nevada is below the national rate, and psychiatrists number 8.7 per 100,000 versus 12.2 nationally. UNR Med reporting has placed Nevada 45th among states for active physicians and 48th for primary care physicians per capita.
About 64.9 percent of Nevadans live in a federally designated primary care HPSA and 91.3 percent live in a mental health HPSA (15 of 17 counties are single-county mental health HPSAs). The state Primary Care Office notes all 17 counties carry some shortage designation; in cities a key driver is that many providers do not accept Medicaid, and in rural and frontier areas travel time to a provider can be several hours. Nevada’s uninsured rate was 11.4 percent in 2024 (47th of 50 states; US 8.2 percent).
Telehealth makes the most sense for Elko-area miners and their families on rotating schedules, ranchers and workers in Spring Creek, Carlin, Wells and Jackpot who face a long drive for every visit, parents who need a pediatric check for a child five or older, and adults who have never been able to get in with a psychiatrist or dermatologist locally. It is also useful for seniors who want regular medication reviews without winter travel.
Emergencies still need the ER. Go to Northeastern Nevada Regional Hospital, or call 911, for chest pain, stroke signs, serious injury on the job, severe breathing trouble or a mental health crisis. For in-person primary and dental care on a sliding fee, Nevada Health Centers’ Elko Family Medical & Dental accepts walk-ins.
Medicaid: Nevada Medicaid and Nevada Check Up (CHIP) are run by the Nevada Division of Health Care Financing and Policy under the Nevada Health Authority. Effective January 1, 2026, Medicaid managed care expanded from Clark and Washoe counties to all 17 Nevada counties; the state expected roughly 75,000 rural members to move into managed care (about a 10 percent increase). Roughly 126,000 people statewide remain in fee-for-service Medicaid, including children in foster care, juvenile justice and child welfare systems, people with disabilities, seniors 65 and older, and people served through Nevada’s three home- and community-based waiver programs. Rural members were auto-assigned to a plan by ZIP code, with a plan-selection deadline of December 26, 2025; members can still change plans within the state’s rules. Medicaid eligibility rules (work, school and volunteer requirements) are scheduled to change January 1, 2027.
Five managed care organizations hold 2026-2030 contracts: Anthem Blue Cross and Blue Shield Healthcare Solutions (Elevance; the award notice listed it as Community Care Health Plan of Nevada), CareSource Nevada, Health Plan of Nevada (UnitedHealthcare), Molina Healthcare of Nevada, and SilverSummit Healthplan (Centene). Per the March 2025 intent-to-award, CareSource and SilverSummit serve Clark, Washoe and all rural counties; Anthem and Molina serve Clark and Washoe; Health Plan of Nevada serves Clark County only. Rural members (including Carson City, Elko and Nye counties) therefore choose between CareSource and SilverSummit.
