Michigan’s largest city, growing again after decades of decline, with the state’s densest concentration of hospitals but deep neighborhood-level shortages of primary and mental health care.
Detroit has more hospital capacity than anywhere else in Michigan, yet getting a routine appointment is still hard in many neighborhoods. HRSA lists 18 primary care and 19 mental health shortage designations in Wayne County, including low-income shortage areas covering east, west and southwest Detroit. A video visit with Charles Wade, FNP-BC, lets you handle primary care, mental health, ADHD, weight management, skin problems and sick visits from home or a break room.
He can order blood work and imaging to be done near you and send prescriptions to the pharmacy you already use. If something needs hands-on care, he will say so and point you to a clinic or emergency department in the city rather than guess.
HRSA data (downloaded October 2026) show 18 designated primary care HPSAs and 19 mental health HPSAs in Wayne County, including low-income population HPSAs for East Detroit (mental health score 20), West Detroit (21) and Southwest Detroit (16); Detroit FQHCs carry primary care HPSA scores of 20-24. ACS 2024 1-year: 7.5% of Detroit residents uninsured; 7.3% of households with no internet subscription and 90.1% with broadband (older 2013 data showed about 40% without internet). Rite Aid’s 2024 exit removed pharmacies citywide.
Telehealth fits Detroit adults and children ages five and older who need a dependable prescriber for blood pressure, diabetes, asthma, anxiety, depression or ADHD and cannot easily take time off or cross the city for a visit. Detroit’s asthma burden is far above the state average, so asthma check-ins and refills are a common reason to book. It also works for shift workers who need follow-up outside business hours.
Go in person instead when you need a physical exam, a procedure, vaccines or a child’s blood lead test, which the Detroit Health Department offers. Call 911 or go to an emergency department such as Henry Ford Hospital, Detroit Receiving or Sinai-Grace for chest pain, trouble breathing, stroke signs, severe injury or thoughts of harming yourself.
Medicaid: Michigan Medicaid is administered by the Michigan Department of Health and Human Services (MDHHS). Most beneficiaries are enrolled in managed care through the Comprehensive Health Care Program (CHCP); MDHHS posts the FY2026 CHCP contract and amendment on its Medicaid Health Plans page. The Healthy Michigan Plan is Michigan’s Medicaid expansion coverage for low-income adults. MI Coordinated Health, a new program for people with both Medicare and Medicaid, launched January 1, 2026 in select counties, replacing MI Health Link, with statewide expansion planned for 2027 (MDHHS newsroom, Oct. 9, 2024).
Nine Medicaid health plans hold CHCP contracts that took effect October 1, 2024 and run through September 30, 2029 with three optional one-year extensions: Aetna Better Health of Michigan, Blue Cross Complete of Michigan, HAP CareSource, McLaren Health Plan, Meridian Health Plan of Michigan, Molina Healthcare of Michigan, Priority Health Choice, UnitedHealthcare Community Plan and Upper Peninsula Health Plan (MHA summary of the April 8, 2024 award; Meridian press release). Plans vary by county per the MDHHS ’Michigan Medicaid Health Plan Listed by County’ (updated 1-10-2025): Wayne County has eight plans (all except Upper Peninsula Health Plan); Kent has six (Blue Cross Complete, Molina, McLaren, Priority Health Choice, Meridian, UnitedHealthcare); Ingham has four (McLaren, UnitedHealthcare, Aetna, HAP CareSource); Washtenaw has six (Blue Cross Complete, McLaren, Meridian, UnitedHealthcare, Aetna, HAP CareSource); Genesee has six (Blue Cross Complete, Molina, McLaren, Meridian, UnitedHealthcare, HAP CareSource); Kalamazoo has four (McLaren, Meridian, UnitedHealthcare, Aetna); Alpena has four (Blue Cross Complete, Molina, McLaren, Priority Health Choice); Marquette and the rest of the Upper Peninsula have one (Upper Peninsula Health Plan).
