Pike County seat and the medical center of far eastern Kentucky’s coalfields, home to the University of Pikeville and the state’s only Level II trauma center, in a county that has lost nearly a quarter of its people since 1990.
Pikeville punches far above its size. Pikeville Medical Center on Bypass Road is Kentucky’s only Level II trauma center and draws patients from across the coalfields and into West Virginia, and the University of Pikeville trains osteopathic doctors for the region. Getting a routine appointment is harder. HRSA scores Pike County’s low-income primary care shortage at 21, among the higher scores in the state, and the entire county is a mental health shortage area.
Families in Elkhorn City, Belfry or Phelps can spend an hour each way on mountain roads for a fifteen-minute visit. A video visit with Wade’s Care First handles primary care, depression and anxiety, ADHD and weight management from home, and sends labs to Pikeville.
HRSA designates Pike County’s low-income population a primary care HPSA (LI-Pike County, score 21) and the whole county a geographic mental health HPSA (score 18, designated 2025); Pike Service Area has been a Medically Underserved Area since 1978, and two Pikeville rural health clinics hold their own HPSA designations. Pike County’s population fell from 72,583 in 1990 to an estimated 54,721 in 2025. Walgreens’ Kentucky store list includes no Pikeville store; the nearest listed town is Prestonsburg.
Sandy Valley Transportation Services runs $1 dial-a-ride public transit and Medicaid rides across Pike and neighboring counties. County Health Rankings (2025) puts Pike County life expectancy at 69.3 years, versus 73.3 for Kentucky. CDC PLACES estimates 10.5% of Pike County adults lacked reliable transportation in the past year.
Pike County carries the health load of a coal county. CDC PLACES estimates 15.6 percent of adults have diabetes, 13.2 percent have COPD, nearly one in four smokes and 45 percent have high blood pressure, and about one adult in ten went without reliable transportation in the past year. Telehealth fits retired miners and their spouses managing blood pressure, diabetes and breathing problems, adults in hollows far from town who want depression or anxiety treatment without the drive, and University of Pikeville students who need a prescriber. A good cell or broadband signal matters, so plan the visit where yours is strongest. Go in person for chest pain, stroke symptoms or serious injury; call 911 or go to Pikeville Medical Center.
Kentucky Medicaid, state-specific plans and HMO plans are not accepted. Participation is limited to the PPO plans and TRICARE listed above, subject to benefits verification.
Medicaid: Kentucky Medicaid is run by the Department for Medicaid Services in the Cabinet for Health and Family Services. It covers low-income children, pregnant women, people with disabilities and older adults, and, under the Affordable Care Act expansion, adults 19-64 with incomes up to 138% of the federal poverty level. The Kentucky Children’s Health Insurance Program (KCHIP) covers uninsured children under 19 who do not qualify for Medicaid, in families up to about 218% of the poverty level. Kentucky SKY, run statewide by Aetna Better Health of Kentucky since 2021, covers foster youth, former foster youth and other children in state care. Most members are enrolled in a managed care plan; the rest are in fee-for-service Medicaid. House Bill 2, enacted in 2026 after the General Assembly overrode the governor’s vetoes on April 14, 2026, makes coverage for many expansion adults 19-64 depend on 80 hours a month of work, school, training or volunteering by January 1, 2027, the same date federal law requires, with exemptions for pregnant and postpartum members, people with disabilities or medical frailty, and parents or caretakers of children under 14.
Five Medicaid managed care organizations serve Kentucky in 2026: Aetna Better Health of Kentucky, Humana Healthy Horizons in Kentucky, Passport Health Plan by Molina Healthcare, UnitedHealthcare Community Plan and WellCare of Kentucky. Anthem Kentucky Managed Care Plan stopped being a Kentucky Medicaid MCO on January 1, 2025, under a court order, and its members moved to other plans. A 2026 state law (KRS 205.5321) extends the current MCO contracts through December 31, 2028. An August 2026 DMS enrollment count showed WellCare with about 376,000 members, Passport about 258,000, Aetna about 216,000, Humana about 191,000 and UnitedHealthcare about 147,000, with about 161,000 in fee-for-service, out of roughly 1.35 million members.
