Graves County seat in the Jackson Purchase of far western Kentucky, still rebuilding after the December 10, 2021 tornado destroyed much of downtown.
Mayfield has one hospital, Jackson Purchase Medical Center on Medical Center Circle, with a 24-hour emergency department. For specialists, most residents drive about 25 miles north to Baptist Health Paducah or Mercy Health Lourdes. The federal government has counted Graves County as medically underserved since 1978, and in 2025 HRSA scored its mental health shortage at 19, one of the higher scores in western Kentucky.
The December 2021 tornado that flattened much of downtown added years of stress, rebuilding and displacement. KentuckyCare on Kings Drive is the sliding-fee clinic in town. A video visit with Wade’s Care First brings primary care, anxiety, depression and ADHD treatment to your kitchen table, with labs and prescriptions sent to Mayfield or Paducah.
HRSA designates Graves County a high-needs geographic primary care HPSA (score 16) and a high-needs geographic mental health HPSA (score 19, designated Oct. 2025), and the whole county has been a Medically Underserved Area since 1978. The December 10, 2021 EF4 tornado killed 24 people in Graves County, destroyed much of downtown Mayfield and the county EMS headquarters; Jackson Purchase Medical Center ran on generators and treated about 240 patients in four days (AHA case study; Louisville Public Media).
There is no fixed-route bus; the Fulton County Transit Authority provides demand-response rides in Graves County, booked by 3 p.m. the business day before. CDC PLACES estimates 9.4% of Graves County adults lacked reliable transportation in the past year.
Graves County’s health numbers track rural western Kentucky. CDC PLACES estimates 14.7 percent of adults have diabetes, 40.5 percent have obesity and 42.7 percent have high blood pressure, and about one adult in eleven lacked reliable transportation in the past year. Telehealth fits farm families and poultry-plant workers who cannot lose half a day to a Paducah appointment, older adults managing blood pressure and diabetes, and anyone still carrying the anxiety, grief or sleep problems that followed the 2021 tornado. Spanish-speaking families can pair video follow-ups with in-person care at KentuckyCare. Go in person for chest pain, stroke symptoms, serious injury or breathing trouble; call 911 or go to Jackson Purchase Medical Center.
Kentucky Medicaid, state-specific plans and HMO plans are not accepted. Participation is limited to the PPO plans and TRICARE listed above, subject to benefits verification.
Medicaid: Kentucky Medicaid is run by the Department for Medicaid Services in the Cabinet for Health and Family Services. It covers low-income children, pregnant women, people with disabilities and older adults, and, under the Affordable Care Act expansion, adults 19-64 with incomes up to 138% of the federal poverty level. The Kentucky Children’s Health Insurance Program (KCHIP) covers uninsured children under 19 who do not qualify for Medicaid, in families up to about 218% of the poverty level. Kentucky SKY, run statewide by Aetna Better Health of Kentucky since 2021, covers foster youth, former foster youth and other children in state care. Most members are enrolled in a managed care plan; the rest are in fee-for-service Medicaid. House Bill 2, enacted in 2026 after the General Assembly overrode the governor’s vetoes on April 14, 2026, makes coverage for many expansion adults 19-64 depend on 80 hours a month of work, school, training or volunteering by January 1, 2027, the same date federal law requires, with exemptions for pregnant and postpartum members, people with disabilities or medical frailty, and parents or caretakers of children under 14.
Five Medicaid managed care organizations serve Kentucky in 2026: Aetna Better Health of Kentucky, Humana Healthy Horizons in Kentucky, Passport Health Plan by Molina Healthcare, UnitedHealthcare Community Plan and WellCare of Kentucky. Anthem Kentucky Managed Care Plan stopped being a Kentucky Medicaid MCO on January 1, 2025, under a court order, and its members moved to other plans. A 2026 state law (KRS 205.5321) extends the current MCO contracts through December 31, 2028. An August 2026 DMS enrollment count showed WellCare with about 376,000 members, Passport about 258,000, Aetna about 216,000, Humana about 191,000 and UnitedHealthcare about 147,000, with about 161,000 in fee-for-service, out of roughly 1.35 million members.
