Illinois’s southernmost county at the Ohio-Mississippi confluence; county seat Cairo; the steepest population loss of any U.S. county 2010-2020 and no hospital since 1986.
Alexander County, at the southern tip of Illinois, has about 4,900 residents, and more than a quarter are 65 or older. Cairo’s hospital closed in 1986 and was never replaced. HRSA designated the whole county a geographic primary care shortage area in September 2025 with a score of 21, placed it in a mental health shortage area in October 2025, and has listed it as a Medically Underserved Area since 1978.
The state public aid office in Cairo closed, and the town has struggled to keep a grocery store open. A video visit with Charles Wade, FNP-BC, brings primary care, mental health, ADHD, weight and sleep care to your phone, with orders sent to facilities you can actually reach.
HRSA designated all of Alexander County a geographic primary care Health Professional Shortage Area on September 11, 2025 with a score of 21, placed it in the Southern IL mental health geographic HPSA (October 2025), and has listed the Alexander County MUA since 1978. Cairo’s hospital closed in 1986; the state closed Cairo’s public aid office, sending residents to Mounds in Pulaski County; the Delta Center behavioral health provider closed during the state budget impasse before reopening under new ownership; the town’s only grocery closed in 2015, a replacement opened in 2023 and closed in 2025; federal public housing (Elmwood and McBride) was closed and demolished from 2017-2019. The county is aging (26.2% aged 65+) and deaths outpace births.
Telehealth helps most for Alexander County residents who would otherwise drive an hour each way for a routine visit: older adults managing blood pressure, diabetes or arthritis pain, people needing mental health or ADHD follow-up, and families with children five and older who need a sick visit. Community Health & Emergency Services clinics and the Southern Seven county clinic remain your in-person options for exams and vaccines.
Do not rely on video for chest pain, stroke symptoms, severe bleeding or breathing trouble; call 911, because the nearest emergency departments are in Carbondale or Cape Girardeau. You will need a phone signal or home internet for a video visit; if yours is unreliable, ask about a phone-only option.
